Enrollment agreement
Review the terms before signing.
This agreement is between the adult parent or legal guardian who signs it ("Guardian") and WISE ZEBRA MICHIGAN ACADEMY L.L.C., a Michigan limited liability company ("Wise Zebra"), located at 4121 Okemos Road, Suite 23, Okemos, Michigan 48864, for the student identified in the enrollment review ("Student") and every in-person enrollment item listed in the signed review. Only Wise Zebra and the signing Guardian are parties to this agreement. A payer, emergency contact, authorized pickup person, or other family member does not become a party unless that person separately signs. The itemized program names, delivery format, quantities, schedules, session or cycle descriptions, unit prices, subtotals, appointment preferences, and enrollment total shown before signing are part of this agreement.
After submission
Signed request · Pending payment and acceptance
The Guardian receives one Enrollment Reference confirming submission.
After payment confirmation and acceptance
Accepted · Enrollment active
A new Student receives a permanent Student ID and final confirmation.
1. Agreement status and enrollment activation
The Guardian's electronic signature submits a Signed Enrollment Request - Pending Payment and Acceptance and confirms agreement to the terms that will govern the enrollment if Wise Zebra accepts it. Signing alone does not obligate the Guardian to pay, reserve a seat, or activate enrollment.
Wise Zebra assigns internal agreement and invoice records and gives the Guardian an Enrollment Reference confirming receipt of the form. If payment is not received by the payment deadline communicated for the selected program, Wise Zebra may close the pending request or release the requested seat.
The enrollment and its selected items, packages, appointments, and reserved seats become active only after Wise Zebra confirms full payment and accepts the enrollment. Acceptance becomes effective when Wise Zebra sends the final enrollment confirmation to the Guardian's provided email address.
If Wise Zebra receives payment but does not accept the enrollment or cannot provide the selected program, Wise Zebra will initiate a refund of all amounts received for that enrollment within ten business days. Additional processing time imposed by a bank or payment provider is outside Wise Zebra's control.
A new Student receives one permanent Student ID in the format WZMA000001 only after payment and acceptance are confirmed. A returning Student's existing Student ID remains the same. A Student ID is a record reference and is not a password.
2. Program, tuition, and payment
Every selected item shown in the final review, including its program name, delivery format, quantity, schedule, number and length of sessions or cycles, unit price, subtotal, and applicable appointment preferences, is incorporated into this agreement. The item subtotals together equal the enrollment total and invoice amount.
Wise Zebra does not currently charge a monetary late fee. A missed payment deadline may cause an unconfirmed seat or appointment to be released.
A Guardian paying by Zelle should enter the Enrollment Reference in the payment memo. For an in-person payment, staff will use the Enrollment Reference to locate and record the payment.
The Guardian is responsible for checking the itemized enrollment summary and contacting Wise Zebra before payment if the Student, any selected item, delivery format, quantity, schedule, session or cycle count, unit price, subtotal, or enrollment total is incorrect.
3. Packages, appointments, and use of sessions
Programs and appointments must be scheduled in advance and are subject to teacher, room, and program availability. Each selected item and package is for the enrolled Student and may not be shared with another student.
The enrollment summary will state any fixed dates, scheduling window, or package expiration that applies. A term not shown before signing or in a later signed addendum is not added merely by an informal message.
Wise Zebra provides scheduled, school-age, subject-specific academic instruction. Supervision ordinarily begins when the Student physically checks in with authorized Wise Zebra staff for the scheduled program and ends when the Student is released under Section 8. Wise Zebra does not provide routine before- or after-class care, transportation, or open-ended drop-off supervision.
4. Cancellation, withdrawal, refunds, and academy cancellations
The Guardian may cancel enrollment by written notice at least seven calendar days before the first scheduled class.
A timely pre-start cancellation receives a refund of tuition actually paid, less disclosed, actual, nonrecoverable payment-processing fees, to the extent permitted by law and payment-provider rules. Wise Zebra will initiate the refund within ten business days after receiving the written cancellation notice. Additional processing time imposed by a bank or payment provider is outside Wise Zebra's control.
When notice is received fewer than seven calendar days before the first scheduled class, Wise Zebra may deduct the lesser of (a) documented, nonrecoverable reservation costs, excluding any separately disclosed payment-processing fee under Section 4.7, or (b) the prorated value of one tuition-bearing session. Wise Zebra will initiate any remaining refund within ten business days after receiving the written cancellation notice. Additional processing time imposed by a bank or payment provider is outside Wise Zebra's control.
After instruction begins, every session that has already occurred is nonrefundable, whether attended, missed with notice, or recorded as a no-show.
Future scheduled sessions that have not occurred may be refunded after Wise Zebra receives written withdrawal notice. Withdrawal is effective when the notice is received. A class that has already started or occurred before receipt counts as used. Wise Zebra will calculate and initiate any resulting refund within ten business days after receiving the written withdrawal notice, subject to Section 4.10. Additional processing time imposed by a bank or payment provider is outside Wise Zebra's control.
Refund calculations are made separately for each selected item. For a fixed ten-session package, the prorated per-session rate is that item's package subtotal divided by ten. For Prealgebra and Introduction to Algebra, tuition is allocated to the twelve core classes. The included Practice and Review sessions have no separate charge and no separate prorated refund value. For any other multi-session or multi-cycle item, the prorated rate is based on the tuition-bearing sessions or cycles shown in the signed enrollment review and repeated in the final enrollment confirmation.
Actual, nonrecoverable payment-processing fees are not refundable when the fees were disclosed before payment and the deduction is permitted by law and payment-provider rules.
If Wise Zebra cancels a class, the class does not count as used. Wise Zebra will first offer a reasonable makeup class. If no reasonable makeup is available, or if the offered time materially differs from the original schedule and the Guardian notifies Wise Zebra within three business days after the makeup offer or before the makeup class begins, whichever occurs first, that the Student cannot attend, the Guardian may choose an account credit or a prorated refund for that class. This provision applies only when Wise Zebra cancels or materially reschedules a class. A Student absence or Guardian scheduling conflict is governed by Section 5 and does not create a refund or makeup entitlement.
If Wise Zebra terminates an enrollment because of repeated or serious Student conduct, tuition allocated to unused future sessions will be refunded under Sections 4.5 and 4.6.
Refunds are issued to the original payer and original payment method when reasonably practical. If that method is unavailable, Wise Zebra may use another verified method agreed with the payer.
Written cancellation or withdrawal notice is effective when received at wisezebrami@gmail.com or through another written notice method expressly designated by Wise Zebra. Wise Zebra will acknowledge receipt electronically.
5. Attendance, absences, late arrival, and no-shows
A no-show occurs when the Student does not attend and Wise Zebra receives no notice before the scheduled class start time.
A fixed-date class counts as used when it occurs, regardless of attendance, unless Wise Zebra cancels the class.
Wise Zebra does not provide an individual replacement class for a Student absence.
A Student may bring questions from missed content to an available Weekly Practice and Review Session. This is supplemental support, not a private makeup class, and does not extend the program or create a refund entitlement.
A late arrival does not extend the scheduled end time. The Guardian should notify Wise Zebra as soon as reasonably possible about an absence or significant delay.
6. Placement, instruction, and educational outcomes
Wise Zebra may use information such as the Student's recent work, Math Check result, teacher observations, and course readiness to recommend an appropriate starting point or adjust the level of practice.
Instruction may include explanation, guided work, independent practice, correction, review, or enrichment according to the selected program and the Student's needs.
Wise Zebra provides supplemental educational instruction and practice. Its programs do not replace the Student's school instruction, assigned classwork or homework, school-provided accommodations or services, or evaluation and treatment by licensed professionals. The Guardian remains responsible for the Student's school attendance, assigned work, and school-related decisions.
Wise Zebra does not guarantee a particular grade, test score, school placement, competition result, course advancement, or rate of progress. Student outcomes depend on many factors, including attendance, effort, prior knowledge, and continued practice.
Wise Zebra may make reasonable teacher, room, or schedule substitutions when needed. A cancellation or material schedule change by Wise Zebra is handled under Section 4.8.
Wise Zebra does not discriminate in providing its services as prohibited by applicable law. A Guardian may request a reasonable modification, communication aid, or other accommodation by contacting Wise Zebra. Requests will be considered individually in light of the Student's needs, the program, safety, and applicable law.
Wise Zebra is a supplemental education provider and does not undertake the obligations of a public school or promise to implement an IEP or Section 504 plan in full. Wise Zebra will securely consider relevant information the Guardian chooses to provide when evaluating a reasonable accommodation or appropriate instructional support.
7. Student conduct, supervision, and emergency response
Students are expected to follow staff directions, use respectful language, care for academy property, and avoid running, pushing, fighting, horseplay, harassment, threats, intentional property damage, unsafe use of materials or equipment, or other seriously disruptive conduct.
Wise Zebra may pause a Student's participation and contact a Guardian when staff reasonably believes immediate action is needed for safety or to restore an appropriate learning environment.
Repeated or serious conduct concerns may result in a required behavior plan, suspension, or termination of enrollment. When required by applicable law, Wise Zebra will consider reasonable modifications before suspending or terminating a Student for conduct that may be related to a disability, unless an individualized assessment determines that the conduct presents a direct threat that cannot be reduced by reasonable modification or that the requested modification would fundamentally alter the program. Tuition allocated to unused future sessions will be handled under Section 4.
The Guardian must provide current contact information and essential allergy, health, mobility, communication, or emergency information reasonably needed for safe participation through the secure process designated by Wise Zebra. Detailed medical records must not be submitted through the public enrollment form or ordinary email. If additional records are reasonably necessary, Wise Zebra will provide or designate a secure submission method.
Wise Zebra does not administer routine medication. Emergency medication may be possessed, stored, or administered only under a separate written medication plan accepted by Wise Zebra and consistent with staff training, insurance requirements, and applicable law.
In an emergency, Wise Zebra may provide reasonable basic first-aid assistance within staff training, contact emergency services, share information reasonably needed for emergency response, and attempt to reach the Guardian and emergency contact. The Guardian remains responsible for charges imposed by outside medical or emergency providers.
Nothing in this agreement requires the Guardian or Student to release a claim or waive liability that cannot lawfully be waived. The Guardian is responsible only for documented direct loss caused by the Student's intentional or willful misconduct, and only to the extent the Guardian is legally responsible under applicable law or later agrees in writing after the incident. This responsibility does not apply to loss caused by Wise Zebra's own negligence or other conduct for which liability cannot lawfully be waived.
Nothing in this agreement limits any person's right or legal duty to contact emergency services, Children's Protective Services, law enforcement, or another appropriate authority. Wise Zebra and its personnel will make reports required by applicable law. Advance Guardian consent or notice is not required when a report is legally required or when notice could increase risk or interfere with a safety response.
8. Pickup, dismissal, and late pickup
Students may check in no earlier than ten minutes before the scheduled class start time unless Wise Zebra has approved another arrangement in writing. The Guardian must not leave a Student before check-in has been accepted by authorized staff. Wise Zebra's supervision begins when the Student physically checks in with authorized staff for the scheduled program. It does not begin while the Student is in a parking area, hallway, lobby, or other location without staff check-in. Supervision ends when the Student is released to an authorized person or under a valid independent-release authorization.
Students under age 12 must be released to a parent, legal guardian, or individual age eighteen or older specifically authorized in the enrollment record or a later verified written update.
Students ages 12 through 17 may leave independently only when a Guardian has provided written authorization. Wise Zebra may decline independent release when staff reasonably believes it would create an immediate safety concern.
The Guardian represents that the Guardian has authority to enroll the Student and provide pickup or independent-release instructions. The Guardian must promptly provide any current court order or other legally sufficient documentation relevant to release. A request by one parent or guardian to restrict release to another legal parent will be honored only when supported by a current court order or other legally sufficient documentation. Wise Zebra may rely on the most recent verified written records it has received and does not adjudicate custody disputes. If a conflict arises at dismissal, Wise Zebra may keep the Student supervised while contacting law enforcement or another appropriate authority.
An emergency contact is not automatically authorized to pick up the Student unless that person is also listed as an authorized pickup adult. Wise Zebra may request identification before release.
A temporary pickup change must be communicated through a method Wise Zebra can reasonably verify. A child's oral statement alone is not sufficient to change the authorized release plan.
Independent-release and pickup authorizations must be confirmed for each new enrollment or renewal and updated whenever the authorized release arrangement changes.
Pickup is due at the scheduled class end time. A brief supervised dismissal period may be used for safe release, but it is incidental to dismissal and is not an aftercare service. After ten minutes, staff will contact the primary Guardian. After twenty minutes, staff may contact the second Guardian or emergency contact.
After thirty minutes without contact, an administrator will evaluate whether contacting local law enforcement or child-protection authorities is reasonably necessary for the Student's safety.
A Student awaiting pickup will remain supervised and will not be left outside or alone.
Repeated late pickup may result in a required pickup plan, suspension, or termination of enrollment. Wise Zebra does not currently charge a late-pickup fee.
9. Records and privacy
This enrollment form must be completed by an adult parent or legal guardian. Wise Zebra uses enrollment information to administer programs, support instruction, protect Student safety, communicate with the Guardian, maintain financial records, and meet legal obligations.
Wise Zebra may use service providers for website hosting, email, recordkeeping, and payment processing. Information will be limited to what is reasonably needed for those services or required by law.
Wise Zebra maintains records in company-managed systems and through contracted service providers subject to appropriate access controls. Email is a communication channel; relevant email messages, system logs, and backup copies may also be retained when reasonably necessary under this agreement, an investigation, an insurance matter, or a legal hold.
Financial and tax-support records are retained for seven years after the related tax return is filed. Signed agreements and core Student records, including attendance, pickup authorizations, and incident or safety records, are retained until the later of seven years after final service or the Student's nineteenth birthday.
Unfinished drafts are ordinarily deleted from active systems within thirty days, and non-enrolled inquiry records are ordinarily deleted from active systems within ninety days, subject to finite backup cycles, consented follow-up, security logs, applicable law, and legal holds. Records involving alleged abuse, sexual misconduct, grooming, serious injury, a child-protection or law-enforcement report, an insurance claim, a threatened claim, an investigation, or a legal hold are excluded from the ordinary deletion schedule. Such records are retained until an authorized Wise Zebra manager or records custodian approves release of the hold in writing after considering applicable law, limitation periods, insurer requirements, and advice of legal counsel when appropriate.
Enrollment, payment, schedule, safety, and instructional messages are service communications. Marketing messages require separate consent when consent is required and are not a condition of enrollment.
Online assessment, student portal, and other child-facing online-service data are governed by the applicable privacy notice and consent presented at the time of collection. If that notice specifies a shorter retention period than Section 9.4, the shorter period applies to those data.
10. Electronic records, signature, and copies
By signing electronically, the Guardian confirms: "I have reviewed this Enrollment Agreement, the enrollment summary, and each required confirmation displayed in the signing form; the information I provided is accurate; I am authorized to enroll the Student; and I intend my typed legal name and submission to serve as my electronic signature."
Wise Zebra will record the agreement version, Enrollment Reference, typed legal name, signer relationship, Michigan local signing time, UTC audit time, and an integrity-protected snapshot of the signed text. A copy capable of being retained, saved, and printed will be emailed to the Guardian after submission. A final confirmation will be sent after payment and acceptance are confirmed.
To enroll electronically, the Guardian needs an email address and a device capable of displaying a web page and opening or saving a PDF. The Guardian may request a free paper copy or staff-assisted enrollment by contacting Wise Zebra before signing.
Electronic-delivery consent applies to this enrollment and its related confirmations, receipts, service notices, and mutually accepted addenda. A future enrollment or renewal requires separate acceptance.
The Guardian may update an email address, request a paper copy at no charge, or withdraw consent for future electronic delivery by writing to wisezebrami@gmail.com . Withdrawal does not cancel an agreement already signed or an active enrollment.
"Required confirmations" means only the confirmations displayed in the enrollment review and preserved with the signed snapshot. Any separately incorporated document will be identified by title and version before signing.
11. Complete agreement, changes, and michigan law
This agreement, the enrollment review, and any later written addendum accepted by both parties contain the terms for the selected enrollment. A material change to price, session count, cancellation rights, or program commitment must be shown in a new agreement or written addendum.
If one provision is found unenforceable, the remaining provisions remain in effect to the extent permitted by law. A delay in enforcing a provision is not a waiver of the right to enforce it later.
This agreement is governed by Michigan law. Nothing in it waives a nonwaivable right or remedy available under applicable law.
The Guardian should ask questions before signing. Wise Zebra will provide a reasonable opportunity to review the agreement and obtain a copy.
I have reviewed and agree to the program, payment, cancellation, refund,
package, attendance, absence, and no-show terms.
I have reviewed and agree to the conduct, supervision, emergency-response,
pickup, dismissal, and late-pickup terms. I authorize Wise Zebra to follow the
emergency-contact and emergency-response procedures described above when
reasonably necessary for student safety.
I have reviewed and agree to the privacy, service-communication, record
retention, electronic-record, and electronic-signature terms.
